Mental Health Facilitator - Behavioral Safety Team - 28 Hours/Week, Nights

PURPOSE OF POSITION:

To improve patient outcomes by assisting allmembers of the health care team with direct patient and consultation to addressthe development and implementation of behavior and safety plans for patients.Role model and promote collaboration and communication among members of the healthcareteam.Work towards strategies tostabilize behavior that will lead to optimal clinical and resource outcomeswith timely discharges. To monitor, analyze, and report on resource utilizationduring the provision of patient care. To assist the healthcare team withtraining parents, staff, care providers and community agencies in individualtreatment plans and crisis intervention for patients in our medical center.

Responsibilities

  • Compliance - Patient ServicesStaff

  • Sustain a working understandingof regulatory requirements and accreditation standards. Support leadership inCompliance efforts to meet and sustain the regulatory compliance efforts of thedepartment and

    hospital. Be mindful andjudicious in monitoring safety and departmental policies and procedures. Ensureall individual certifications and Safety College requirements are fulfilled ina timely basis. Work with department managers, nurse council and magnetrepresentatives to identify and share safety best practices. Be mindful ofCCHMCs processes and compliance with CMS standards and freely communicatesafety and compliance concerns to leadership team members.

    Coordinates care of patentsthrough appropriate patient placement and care provider assignments.Effectively communicates clinical information concerning patients. Collaborateswith other healthcare providers promoting smooth transitioning to differentlevels of care. Monitors staffing levels and acuity needs. Facilitates patientprocessing by collaborating with family, interdisciplinary team members anddischarge support staff.Facilitatesprocurement of services and resources.

    Facilitates the involvement of patients/families andproviders in care decisions, goal setting and

    discharge planning. Assists withthe coordination of multi-departmental resources and provides support tohealthcare providers as it relates to the discharge process.

    •Resource

    Serves as a resource forinterventions to improve clinical outcomes. Participates in improvementactivities and provides input in department discussions focused on improvingpatient care.

    •Additional Training/Education (home unitspecific)

    DBT, Relationship based care model,TCI, Child and or Adult enhancement model.



    Qualifications

     

    QualificationsRequired:

    ·Bachelor'sDegree in a related field (Social Work, Counseling, Psychology, SpecialEducation, etc.

    ·2 years related experience working directly withbehaviorally complex patients.Mostnotably working with children who may have the following: Autism SpectrumDisorder, Developmental Delay, Oppositional Defiant Disorder, Psychosis.


    Preferred:

    -Person who can adapt to complex situations and respondeffectively in crisis situation.

    -Person who has the ability to learn about medicalcomplexities and have the ability to collaborate with medical teams bydiscussing patient behavior and subsequent interventions utilized for patientcare.

    -Person who can provide direct behavioral support to thebehavioral needs of patients.

    -Person who can communicate behavioral concepts clearly toothers who may not have behavioral knowledge.

    Capableof sitting, standing, walking, bending, stooping, kneeling, reaching, hearing,twisting, seeing, speaking, grasping and or manipulating objects and climbing.Capable of lifting and pushing up to 75 lbs. Capable of carrying up to 50 lbs.Capable of managing physically aggressive patients.
