  • JOB RELATIONSHIPS
    • Responsible to the Director of Patient/Quality Services. Consults with members of the Patient Care Services Leadership and Hospital Leadership staff as appropriate.
  • DEFINITION OF POSITION
    • The Medical Social Worker or Patient Family Services Case Manager is a Masters or 4 year undergraduate prepared individual who is responsible for facilitating social work or social work-related services to patients and their families, who utilize both inpatient and outpatient medical services, in conjunction with interdisciplinary health care team members.
  • QUALIFICATIONS
    • Professional
      • Master of Social Work degree from a program accredited by the Council on Social Work Education preferred, other masters degree in human services field or 4 year undergraduate degree in social work, psychology or other human services degree accepted..
      • Two to three years of post-masters/post-bachelors experience in a health care setting such as a hospital, mental health setting, extended care facility, or home care agency.
      • Knowledge of applicable legal responsibilities/restrictions and current trends in social work/social work-related services pertinent to the hospital practice setting.
      • Knowledge of acute, chronic and terminal illnesses, mental and physical disabilities, and the normal developmental process and the impact on family systems.
      • Evidence of participation in ongoing continuing education programs.
      • Knowledge of and adherence to the National Association of Social Workers Code of Ethics.
    • Personal
      • Maintains the physical and emotional stability necessary to perform job tasks.
      • Demonstrates an ability to relate effectively to patients and their families and other interdisciplinary team members and maintain a professional working relationship.
      • Effective oral and written communication skills.
      • Evidence of knowledge and ability to use proper channels of communication.
      • Maintains a professional appearance.
    • Physical
      • Stamina Minimally capable of sitting/standing/walking for long periods of time (2-4 hours).
      • Strength Minimally capable of free lifting 20 pounds.

      • Reflexes/Agility Minimally capable of:
        • Stooping, kneeling, reaching, bending, and twisting.
        • Responding quickly and effectively to emergencies, i.e., fire.
      • Hearing and Visual Acuity
        • Capable of hearing and responding to patients, their families, and staff members both verbally and via telephone.
        • Capable of visual acuity that permits reading, writing, patient assessment, and monitoring office equipment, i.e., computer, fax and copy machine.
      • Dexterity Possesses handwriting skills to facilitate patient documentation and correspondence and capable of handling office machines i.e., computers, copy machines, etc.
