The Community Outreach and Program Enrollment (COPE) Specialist works under the general direction of the Director of Programs to build authentic relationships with consumers through community outreach and telephonic intervention. In collaboration with Options Counselors, Information Specialists and the Admissions Team, the COPE Specialist will play a key role in providing consultation, education and program enrollment assistance to older adults, people with disabilities and their loved ones as well as caregivers. To that end, the COPE Specialist will significantly increase the number of people served across all BSHC programs by developing and implementing a multi-faceted outreach and enrollment strategy to reach seniors and people with disabilities throughout the City of Boston. You will also have ample opportunities to actively participate in BSHC's organizational culture.

ESSENTIAL FUNCTIONS

Duties 1-14 are designated as ADA essential functions and must be performed in this job. All other job duties are secondary functions.

  • Assess and triage telephone calls from consumers seeking community resources and referrals, program and enrollment information, disabled care information, and other community services and supports
  • Perform preliminary screenings to determine the consumer's eligibility for publicly funded programs and make appropriate referrals
  • Log all calls using a call tracking database
  • Skillfully use resource databases to locate appropriate community resources for consumers
  • Advocate, as needed, on behalf of older adults, people with disabilities and their loved ones as well as caregivers to secure services, including:Exhibit working knowledge of BSHC services/programsContinually assist in the development of community resources knowledge within BSHCSeek peer or supervisory consultation to explore unfamiliar resource areas
  • Outreach to professional organizations to provide information about BEI and BSHC programs and supports
  • Serve as an information resource to all direct service staff at other Aging Services Access Point (ASAP) agencies
  • Participate in trainings and meetings as designated and perform outreach and other administrative duties as assigned by the Director of Programs
  • Identify areas of daily living where support may be needed for consumers
  • Locate housing alternatives (i.e., assisted living, subsidized apartments or senior living communities) as needed
  • Assist consumers and their loved ones with navigating life changes (i.e., loss of income, widowhood, diagnosis of Alzheimer's or other chronic disease, etc.)
  • Help consumers and caregivers plan for future needs (i.e., assist with identifying community activities to help consumers reach their independent living goals and provide supports to consumers with communication and behavioral challenges)
  • Contribute to BSHC's organizational culture through active participation in team activities, committees, and work groups. Promote a positive work environment through cooperative and respectful interaction with others.
  • Using all external contacts with vendors and the community at large, continually participate in outreach efforts as a BSHC representative to ensure access to services for all member groups.
  • COMPETENCIES

  • Knowledge of Boston's neighborhoods, older adult services network, independent living community and other available resources available through the Aging and Disability Resource Center (ADRC)
  • Must be detail-oriented and have strong administrative and organizational skills to perform tasks in a timely manner
  • Sensitivity to older adult and diversity issues
  • Commitment to maintaining members at home with dignity
  • Ability to effectively communicate in person, in writing, and by telephone
  • Demonstrated capability using multiple computer and information systems and platforms
  • Must be able to use Microsoft Office (i.e., Word, Excel, PowerPoint and Outlook)
  • SUPERVISORY RESPONSIBILITY

    This position has no supervisory responsibilities.

    WORK ENVIRONMENT

    This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, copiers, filing cabinets.

  • POSITION TYPE/EXPECTED HOURS OF WORK

    This is a full-time position, Monday-Friday, 9 a.m. - 5 p.m.

    TRAVEL

    This position requires up to 50% travel. Must be able to travel throughout the community under all weather conditions and perform home visits.

    REQUIRED/PREFERRED EDUCATION AND EXPERIENCE

  • Bachelor's degree required in social work, human services, gerontology or related field
  • Experience with telephone information and referral, assessment and problem solving. Must be able to work with callers with complex issues in a professional manner to bring about a successful resolution
