Registered Nurse (RN)- Case Manager - Case Management 001 - Full-Time,Evening - Indianapolis-2001 West 86th St - Indianapolis,IN
Work Schedule: This will be a swing shift from 12-8:30 M-F with occasional weekends and holidays.

What You Will Do
As a Registered Nurse - Case Manager Associate with St. Vincent Health-IN, you will have the opportunity to coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.
Responsibilities:
+ Creates plan for care across the continuum, integrating patient/family preferences and values.
+ Monitors patient care through assessments, evaluations and/or patient records.
+ Advocates for resources and removal of barriers.
+ Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
+ Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
What You Will Need
Licenses/Certifications/Registration:
+ Required Credential(s):
+ One or more of the following:
+ Registered Nurse credentialed from the American Nurses Credentialing Center (ANCC) obtained prior to hire date or job transfer date.
+ Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date.
+ Preferred Credential(s):
+ BLS Provider credentialed from the American Heart Association (AHA).
Education:
+ Diploma
+ Diploma from an accredited school/college of nursing required
Work Experience:
+ Case management experience preferred
